Concierge Package
An all-inclusive approach to health care. With one monthly price for self, couple or family. This allows you to obtain wellness/sick visits with access to a provider, via, text, phone, email or virtual visit based on specific health needs.
Includes;
- Provider assessment with appropriate treatment
- Orders for appropriate labs and or diagnostic testing based on diagnosis. (Orders sent to the lab, diagnostic site of your choice and separately billed by this entity)
- Referrals to specialists based on needs, wellness screenings

Self Price $60/mo
Self + Spouse $100/mo
Family $150/mo
Receive a 10% discount if paid in full for 6 months or 1 year.
Non-concierge basic consultation/appointment $30/visit.
